What is a chronic wound?

A chronic wound is any wound that has not become smaller or healed after 4 weeks of traditional care.  Our clinical team works with your primary doctor or healthcare team for the right individualized treatment plan to get the patient back to living a full, healthy life.  We determine the underlying cause that has prevented a wound from healing, and set the patient on the path for success and prevention of reoccurrence.

  • If the patient also has Home Health, we will work in conjunction with them to ensure consistent and comprehensive care from both providers.

    They will continue to provide home health services while we take over care of only the wound.

We understand the importance of continuity of care, and ensure that patient notes are shared back to the referring provider so all parties are collaborative towards each patient’s healing process. 

When partnering with WoundDirect, your referral to us is for care of only the wound, and we assume treatment and responsibility for only the wound, while your clinic/agency continues treatment for all other aspects of that patient’s care if you want. 

If you are referring from an Acute Care facility and the patient needs Home Health treatment outside of our services, we will write orders and bring in one of our Home Health partners to provide care.  We ensure each patient gets the full care they need, whether it comes from us or another provider.

  • To refer a patient, do any one of the following:

  • Please provide the patient’s:

    • demographic information

    • insurance details

    • all medical history and wound care notes, including photos if available

  • Yes! Please call us, or fill out the online referral form and we will reach out to you directly with next steps

  • We accept many types of insurance, including Medicare Part B and Medicare Advantage plans. Our team will first verify coverage for you before starting treatment of any kind.

    We do not accept Medicaid.

  • Typically 3-5 days. First we verify insurance and may have to gather more information for the insurance provider prior to being approved. Once approved, we will get you on our schedule immediately and our clinical team will provide you with updates and scheduled visit times.

  • Typically once per week. If there is a Home Health company also involved with this patient, we will communicate to them to ensure they can schedule care for the other 6 days of the week, and our practitioners will write notes, communicate treatment plan, and provide regular updates to that Home Health team and any other providers involved in this patient’s care.

  • We primarily employ Nurse Practitioners for advanced care, and they are licensed and trained to provide an escalated level of care that is not available with standard Wound Care providers.

    We use cutting-edge skin grafting technology and other advanced services that can lead to faster healing and better outcomes that traditional conservative wound care.
